Our Mission - PACE Southeast Michigan is a unique health plan and comprehensive care provider, committed to keeping chronically ill aging adults in their home, by caring for their medical, psychosocial, and spiritual needs. Our comprehensive program offers an alternative to nursing homes, while also allowing peace of mind for family and caregivers. PACE Southeast Michigan uses Medicare, Medicaid and private funds to cover all of your medically-necessary care and services. PACE Southeast Michigan provides care and services in you home, community, and our day health centers throughout Wayne, Oakland, and Macomb counties. PACE Southeast Michigan participants receive most of their care from our expert staff at our our centers. We provide transportation to and from our centers and all
medically-necessary appointments.
